Objective: Ambulatory surgical centers (ASC's) have emerged as an alternative to the traditional hospital- based outpatient department (HOPD). We aim to determine the effect of surgical setting on adverse events following anterior cruciate ligament reconstruction (ACLR).Methods: The Humana Claims Database was queried for all patients undergoing ACLR in the HOPD or ASC setting, using the PearlDiver supercomputer. To eliminate selection bias in our study, the HOPD and ASC cohorts were propensity score matched on baseline demographics, comorbidities, and operative factors. Comparisons between the matched cohorts were made using chi-square tests. Logistic regression models were created to determine the effect of surgical setting on adverse events.Results: A total of 13,647 patients were queried in our study, 5,298 of whom underwent surgery in an ASC and 8,349 of whom underwent surgery in an HOPD. Analysis of the post-matched cohort revealed no differences between cohorts for mechanical failure, nerve injury, pulmonary embolism, septic joint, wound infection, revision surgery and readmission. Rates of deep vein thrombosis (1.18% vs 1.84%; p = .03) were significantly lower in the ASC group. On logistic regression, ASC was associated with decreased risk for deep vein thrombosis (.87, .83-.93) and pulmonary embolism (.85, .78-.95).Conclusion: ACLR performed in ASC is associated with reduced risk of venous thromboembolism and no difference in surgical morbidity and readmissions versus ACLR performed in HOPD. Development of a standardized algorithm for patient selection in the ASC setting is needed to preserve acceptability of ASC-based ACLR in cost-savings and patient safety models.

BACKGROUND: The demand for conversion of prior hip surgery to total hip arthroplasty (conversion THA) is likely to increase as a function of increasing US hip fracture burden in addition to its application in managing other conditions. Thus, outcome analysis is warranted to better inform value-based reimbursement schemes in the era of bundled payments. METHODS: Via Current Procedural Terminology codes, the National Surgical Quality Improvement Project data files were queried for all patients who underwent primary THA and conversion of previous hip surgery to THA from 2005 to 2014. To better understand the isolated effect of procedure type on adverse outcomes, primary and conversion cohorts were then propensity-score matched via logistic regression modeling. Comparisons of the study's primary outcomes were drawn between matched cohorts. Statistical significance was defined by a P-value less than or equal to .05. RESULTS: Relative to the primary THA group, the conversion THA group had statistically greater rates of Center Medicare and Medicaid Services (CMS) complications (7.5% vs 4.5%), non-home bound discharge (19.6% vs 14.7%), and longer length of hospital stay. Conversion THA was associated with increased likelihood of CMS complications (odds ratio 1.68, confidence interval 1.39-2.02) and non-home bound discharge (odds ratio 1.41, confidence interval 1.25-1.58). No statistically significant differences in mortality and readmission were detected. CONCLUSION: The elevated risk for CMS-reported complications, increased length of hospital stay, and non-home bound discharge seen in our study of conversion THA indicates that it is dissimilar to elective primary THA and likely warrants consideration for modified treatment within the Comprehensive Care for Joint Replacement structure in a manner similar to THA for fracture.

RESUMO

Interest in outpatient orthopedic surgery has been fueled by provider desire to control costs and development of rapid recovery protocols. Open reduction and internal fixation (ORIF) is a commonly elected treatment strategy for ankle fracture that may be performed in an outpatient setting. Lessons on cost-savings of the outpatient model in orthopedics can be learned in total joint replacement and spine surgery. Moreover, in properly selected patients, outpatient ORIF has been shown to be comparably safe. Reasons for admission of the surgically managed patient with ankle fractures, including concern for surgical delay and additional social factors, warrant further investigation.
